RMA REPAIR / MODIFICATION REQUEST FORM
Important: In submitting this request, I (the user) agree to the following terms and conditions:
- To collect the RMA chargeable item from Repair center within 60 days including receipt of PO and Payment.
- Repair on RMA item with expired warranty are chargeable.
- Repair on RMA item where damage resulted from acts of God, environmental or atmospheric disturbances or other external forces through misuse, abuse, unauthorized alteration or repair are chargeable.
- Please ensure copy of the import invoice is attached with returned unit/Airwaybill document copy emailed.
- If "Part shipment" questionnaire is not answered (Yes/No), we will default to complete shipment only.
- Please ensure all items returned are marked as Returned for "Repair/Modification" and protected by appropriate packaging.
- If a relay is not returned in a case, please refer to packing instructions on Page 3.
- MICOM/Multilin/650 relays built with CT/VT modules must be returned with the heavy-duty terminal blocks attached to ensure relay warranty.
- GE to return Chargeable RMA item to customer if PO not placed after 60 days of offer notification.
- GE to dispose uncollected RMA item after 90 days from first email notification date.
